We are currently looking for a Full time Case Manager Coordinator for our Braddock location.

This position is responsible for assisting in the day to day operations and management of the Center. Oversees case coordination and program services for the site and provides assistance to the Assistant Director and (CTC) Director.

Essential Job Functions:

  • Supervises the work of the Work Release Case Managers.
  • Builds and maintains strong relationships with the Department of Corrections, Parole and all other community stakeholders.
  • Liaise between the community and the center to ensure smooth service delivery, collaboration and cooperation.
  • Assists with the performance/supervision of custody control event reports (urine tests, searches, etc.).
  • Coordinate and reviews referrals for appropriateness of placement.
  • Assists with the submittal of Extraordinary Occurrence reports.
  • Prepares weekly and monthly reports for the DOC.
  • Serves as a member of the program’s interdisciplinary team.
  • Assist with staff training and development in coordination and consultation with the Assistant Director and CTC Director.
  • Assists with reviewing and updating Policy/Procedure Manuals.
  • Assists in the review and update of the Administrative & Operations Manual and Resident Handbook.
  • Maintains a working knowledge of the Department of Health and Department of Corrections/ACA regulations, licensing and standards and assists Assistant Director and CTC Director to ensure that the program is in a state of readiness for PREA/ DOC/ACA visits.
  • Assists in preparation of ACA reaccreditation visits
  • Conducts unannounced visits/walk-throughs to monitor the operation and performance of the facility and the program.
  • Responds to patient grievances and issues discipline as appropriate.
  • Completes work release case manager performance evaluations and annual training plans. Intervenes and documents appropriately when an employee demonstrates poor work performance.
  • Coordinates and communicates with other Gateway programs and departments.
  • Provides direct patient care as needed including lectures, group sessions and crisis intervention.
  • Monitors daily census with the goal of meeting or exceeding ADC projections.
  • Assist with PREA compliance and act as back up PREA Compliance Manager.


3-5 YEARS experience in correction setting or direct case management experience preferred.

Bachelor’s degree preferred
